and 

Info fld=0x1401 st0: Error on write filemark. st0: Current: sense key: Medium Error Add. Sense: Write error 


The first one says the controller is unable to maintain the servo position of the read/write heads over the disc track required. 

The second says the write failed in verification, which is most likely to be a consequence of the first one. 

The common theme is that you have lost, are are about to lose, the disk. You should have a backup regime, but you might just get away with taking a copy right now
www.unixbabuforum.inHave you tried a new good tape? Try writing to the tape using cpio 

find /etc -print|cpio -ocvB >/dev/st0 
Have a look at the "mt" commands in the manual
